YCAAA Introduces Two New Evidence-Based Programs

YCAAA announces the introduction of two new evidence-based programs from Stanford University School of Medicine – Patient Education Research Center!

The programs are from Stanford University's Steps to Healthier LivingTM:

 Chronic Disease Self-Management Program for older adults  living with chronic conditions, and 

• Diabetes Self-Management Program for older adults with type 2 diabetes

Beginning in October, both of these Self-Management workshops will be held in community settings throughout York County, once a week for six weeks. Each workshop spans 2 ½ hours.  Participants will receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, 4th Edition, and an audio relaxation tape. 

Classes are limited to 16 participants. There is no charge for the program.

Chronic Disease Self-Management Workshops:
