The primary focus of the York County Area Agency on Aging is to provide education, advocacy and coordination of community-based services to empower older adults to maximize their independence and quality of life.

The York County Area Agency on Aging offers programs to assist with self management of diabetes and chronic conditions, fall prevention and healthy aging.

Diabetes Self-Management Program
Diabetes does not have to take control of your life!  There are "tools" available to take control of your diabetes in a way that fits your life, your way! 

Living Well With Diabetes -- Your Life, Your Way is a structured, evidence-based program designed by the Self-Management Resource Center to help individuals living with type-2 diabetes manage their diabetes and the emotions that come with the disease.

Looking Beyond My Condition:  Empowerment Tools to Manage Your Health, Your Way

A Chronic Disease Self-Management Program developed by Stanford University, is designed for people living with chronic conditions.

The FREE program is conducted in workshops once a week for six weeks.  Each workshop spans 2 ½ hours.  Participants receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, 4th Edition, and an audio relaxation tape. Follow this link for more information.

A Matter of Balance

A Matter of Balance is an award- winning program designed to reduce the fear of falling and increase the activity levels of older adults who have concerns about falls.  This program utilizes volunteer coaches to teach eight, two- hour classes where participants learn to:

• View falls and fear of falling as controllable
• Set realistic goals for increasing activity
• Change their environment to reduce fall risk factors
• Promote exercise to increase strength and balance

"10 Keys"™ to Healthy Aging

This health promotion program was developed by the Center of Aging and Population Health at the University of Pittsburgh. Their research team, along with the CDC Prevention Research Center, learned a great deal about how to keep adults healthy as they age. They have translated what has been learned into the “10 Keys"™ to Healthy Aging.  Each key is an important step in preventing disease and improving the quality of our lives.

The information is presented in workshop settings, with one or two topics per session, once or twice a week. A certified Health Ambassador through the York County Area Agency on Aging conducts the program.  Participants receive a Resource Manual which provides a wealth of information on preventative, proven strategies that work!
