Services

SUPPORTS COORDINATION
Supports Coordination is a service which provides information and makes referrals for needed services Supports Coordinators work with individuals to plan their services and supports through locating, coordinating and ongoing monitoring.

PRE-VOCATIONAL SERVICES
These services provide supervised job training in a sheltered environment, or at a work site. The focus is on work skills that will help individuals to progress to a level for possible job placement.

COMMUNITY HABILITATION
Community Habilitation provides a planned program of life skill training and work skills. These services are provided to help individuals become more independent at home and in the community.

TRANSITIONAL WORK SERVICES
These services are directed towards supporting individuals in transition to competitive employment through work that occurs in a location other than a facility.

COMMUNITY HOMES
This service provides a supervised home setting for individuals with intellectual disabilities. These settings include group homes, family living and supported living.

RESPITE CARE SERVICES
Respite Care provides short term relief for families. Respite care is provided in the individual’s home or a licensed facility.

HOME & COMMUNITY SERVICES
Direct Services are provided in home and community settings to assist individuals in acquiring, retaining and improving self-help, and adaptive skills.

FAMILY SUPPORT SERVICES
Family Support Services are designed to offer a variety of services to families/individuals, including respite care, family aides, homemakers, recreation and social activities.

ELIGIBILITY FOR INTELLECTUAL DISABILITY SERVICES:
Individuals desiring services must meet eligibility requirements and be a resident of York County or Adams County. Eligibility is based on possessing an IQ of 69 or below prior to the identified individual’s 22nd birthday.
