Living Well with Diabetes

The Diabetes Self-Management Program from the Self-Management Resource Center, formerly Stanford University Patient Education Research Center, is a structured, evidence-based program for older adults living with pre-diabetes or diabetes, which provides tools to manage diabetes and emotions that come with the disease.
The program is conducted in workshops, once a week for 6 weeks. Each workshop spans 2.5 hours. Participants receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, 5th Edition, and an audio relaxation tape.
Who Can Participate?
- Anyone age 60 and older living with pre-diabetes or diabetes
- A caregiver, age 60 and older, caring for someone with diabetes
