Looking Beyond My Condition: Empowerment Tools to Manage Your Health, Your Way is a Chronic Disease Self-Management Program developed by Stanford University, designed for people living with chronic conditions.
The program is conducted in workshops once a week for 6 weeks. Each workshop spans 2.5 hours. Participants receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, 4th Edition, and an audio relaxation tape.
Who Can Participate?
- Any individual age 60 or older, with at least 1 chronic medical condition
- Caregivers, age 60 or older, and family members of those living with a chronic condition
- Those age 60 or older living with cancer or cancer survivors
